Cancellation Policy

All clients must recognize that they are responsible for the charges incurred for the services provided by Jackson. Integrative Health LLC. We do not file insurance on your behalf only accepting payment in full at time services rendered via cash or credit card. By signing below, you agree to these terms and to not attempt to file back with insurance, unless discussed at initial intake as to do so. Missed appointments are a loss for everyone. Cancellations without adequate notice cannot be filled and take valuable time from other clients thus they are subject to a late cancel/no show fee for Physical Therapy appointments and massage appointments at 50% of your appointment total. A credit card will be kept on file for future booking. Therefore, we require that you notify us 24 hours in advance if you’re unable to attend a scheduled appointment. If you are 15 minutes late or greater, the therapist will have the option of seeing you or rescheduling. I understand that I remain personally responsible for the amount due this office for their services. I further understand and agree that this assignment and authorization does not constitute any consideration for the office to await payments and they may demand payment from me immediately upon rendering services at their option.
